Making a Claim
Please note: In 2008, the Australian Netball Risk Protection Program applies ONLY to Netball NT, Netball SA, Netball Queensland, Netball Tasmania, Netball Victoria and WA Netball.
Important notice: If you are member of Netball NSW or Netball ACT you are NOT covered under the 2008 Australian Netball Risk Protection Program. Please contact Netball NSW or Netball ACT for further information regarding your insurance in 2008 and how to lodge a claim.
How To Make A Claim
Step 1
Access a current Personal Accident Claim Form. Claim Forms are available via the following methods:
- Download – by downloading a claim form. You will need to send the claim form to us with any medical accounts for reimbursement;
- Phone – by phoning Echelon on 1800 640 009 (this is a toll free number from anywhere in Australia). Claims staff will send a claim form to you for completion.
Step 2
Complete all relevant sections of the claim form.
- Your claim form may be returned if there is important information missing.
- For assistance contact Echelon on 1800 640 009.
Step 3
Send your claim form to Echelon as soon as possible within 180 days from the date of injury.
- If treatment is likely to continue beyond 180 days, please send through your claim form anyway, with any current receipts received and contact Echelon for further assistance.
Step 4
Echelon will confirm receipt of your claim or make contact with you should they require more information.
- Please contact Echelon directly if you have not received confirmation of your claim after two weeks from the date of lodgement.

Who is Echelon?
Echelon Australia Pty Ltd (Echelon) is a wholly owned subsidiary of Jardine Lloyd Thompson Pty Ltd. Echelon is the appointed claims management group for all Personal Injury claims on behalf of the Insurer and the Trustee of the Netball Australia Risk Protection Program.
We legally can not reimburse you for Medicare-related costs:
The Health Insurance Act (Cth) 1973 does not permit the Insurer or the Trustee to reimburse you for any costs associated with medical treatments registered with Medicare (this includes the Medicare Gap).
Only Non-Medicare Medical Treatments can be reimbursed (as per the Policy Wording):
All treatments must be certified as “necessary” by your physician. i.e. Doctor, physiotherapist, etc.
Loss of Income Claims
How to make a Loss of Income Claim:
To lodge a Loss of Income Claim, please complete Section C of the Personal Accident Claim Form. Claim forms may be obtained via any method as outlined above.
Claims must be lodged within 180 days from the date of injury.
Important information regarding Loss of Income Claims
- The elimination period is 14 days
- The Attending Physicians Report must be completed by the practitioner who attended to you i.e. Doctor/Physiotherapist/Chiropractor; if a fee is charged for completing this section, that fee will be borne by the claimant.
- Your loss of Income claims must be covered by a Medical Certificate for the full period of disability.
- For tax purposes we recommend that you seek the advice of a qualified tax consultant. You may be required to pay tax on any Loss of Income benefit received.
Capital Benefits Claims
How to make a Capital Benefits Claim
If you wish to make a capital benefit claim or there is a potential for such a claim relating to permanent or partial disablement please notify Echelon immediately by phone 1800 640 009. Echelon claims staff will advise you on what you need to do next.
Liability Claims
How to make a Liability Claim
- Liability claims may include Public and Products Liability, Errors and Omissions or Club Management Liability claims.
- It is essential that you notify JLT Sport immediately on 1300 130 373 of any claim or potential claim.
- It is also extremely important not to admit liability or discuss the matter with a third party. JLT Sport claims staff will advise you on what you need to do next.
